Ann's character started as an experiment. I wanted to take a negative concept, in this case, physical pain, and create an anthropomorphic personification for that concept, similar to Terry Pratchett's and Neil Gaiman's characters. I also wanted this character to be a Good person. This led me to consider the difference between natural pain causes, targeted pain causes, and how an immortal linked to the concept of pain would feel about how Humanity has treated its own.
As I further developed her, Ann moved away from her anthropomorphic personification roots and closer to a spirit that had more in common with biblical angels. This inspired me to create another design that would be how she appeared to other spirits and would be the true source of her unusual appearance in her humanoid form.
